What to do right now

Ibuprofen (if you are able to take it) does more for dental pain than paracetamol alone. Follow the packet dosage.

Cold compress on the outside of the cheek, twenty minutes on and twenty off.

Never put aspirin directly on the gum. It burns the tissue and makes treatment harder.

Sleep propped up. Lying flat raises the pressure in the tooth and is why toothache always feels worse at 2 AM.

Avoid very hot, very cold and sugary things until you have been seen.

What we do at the office

A toothache is a symptom, not a diagnosis. The pain usually comes from one of four things: decay that has reached the nerve, a cracked tooth, an infection at the root tip, or a gum problem around the tooth. They feel similar and are treated completely differently, which is why the first thing we do is a digital X-ray.

Once we know which it is, we treat it in the same visit wherever possible — a filling, a root canal to remove the inflamed nerve, drainage if there is infection, or an extraction if the tooth cannot be saved. The priority is always to stop the pain first and discuss the longer-term options once you are comfortable.

Go to a hospital emergency room first if you have swelling closing an eye or spreading into your neck, difficulty breathing or swallowing, or a fever alongside facial swelling. That is a medical emergency before it is a dental one.

How fast can you see me?

Questions

Why does it hurt so much more at night?

Two reasons: lying down increases blood pressure in the head, which raises pressure inside an inflamed tooth, and there is nothing to distract you. This is genuinely why emergency dental calls peak between midnight and 4 AM.

Will antibiotics fix it?

No. Antibiotics can reduce an infection around a tooth, but they do not treat what caused it. The pain usually returns once the course finishes unless the tooth itself is treated.

Can I wait until I get home?

If the pain is mild and there is no swelling, sometimes. If it is waking you up, or your face is swollen, waiting normally means a bigger and more expensive procedure later.
