What to do right now

Keep the crown. Rinse it and put it somewhere safe — if it is intact we can often re-cement the original, which is much cheaper than making a new one.

Never use superglue. It is toxic in the mouth, it sets in the wrong position and it usually ruins both the crown and the tooth underneath.

Temporary dental cement from a pharmacy is fine for a few hours if you cannot get to us immediately.

Chew on the other side and avoid anything very hot, cold or sweet.

If the exposed tooth is sharp, cover it with dental wax.

What we do at the office

An exposed tooth without its filling or crown is sensitive because the dentine underneath is open, and it is fragile because the remaining walls are thin. Left alone for weeks, the tooth commonly fractures or decays where the gap sits, which turns a simple re-cement into a root canal.

If the crown is intact and the tooth underneath is sound, we clean both and re-cement it — usually a short appointment. If the crown is broken or the tooth beneath has decayed, we rebuild the tooth and discuss whether a new crown is needed. Lost fillings are simply replaced.

Go to a hospital emergency room first if you have swelling closing an eye or spreading into your neck, difficulty breathing or swallowing, or a fever alongside facial swelling. That is a medical emergency before it is a dental one.

Can you re-cement a crown that came off months ago?

Sometimes, but the odds fall with time, because neighboring teeth drift into the space and decay starts at the margin. The sooner it goes back on, the more likely the original still fits.

I swallowed the crown. What now?

It is not dangerous and will pass. We will need to make a new one, so bring what information you have about the original.

Is this really an emergency?

Usually not a middle-of-the-night one, unless it is painful or the edge is cutting you. But it is genuinely time-sensitive, and if you are on holiday it is far better dealt with here than left for a week.
