Cold compress on the outside of the cheek. Never apply heat — it encourages the infection to spread.

Keep your head elevated, including when sleeping.

Do not try to lance or squeeze it. If it bursts on its own the taste is unpleasant but not dangerous; rinse with warm salt water.

Old antibiotics from a previous course are not a solution. They are usually the wrong drug, the wrong dose, and they make the infection harder to treat.

What we do at the office

An abscess is a pocket of infection, either at the root tip of a dead tooth or in the gum beside it. The reason it is urgent is not the pain — it is that the infection has somewhere to spread, and the spaces it spreads into are in the floor of the mouth and the neck.

Treatment is drainage plus dealing with the source. We drain the pocket to relieve the pressure, which usually brings immediate relief, then treat the tooth itself with either a root canal or an extraction. Antibiotics are often prescribed alongside, but they are the support act, not the treatment.

Go to a hospital emergency room first if you have swelling closing an eye or spreading into your neck, difficulty breathing or swallowing, or a fever alongside facial swelling. That is a medical emergency before it is a dental one.

How do I know if it has become dangerous?

Swelling that closes your eye, spreads under the jaw or into the neck, difficulty swallowing or breathing, or a fever with the swelling. Any of those and you go to a hospital emergency room, not a dentist.

It burst and the pain stopped. Am I fine now?

No. The pressure is released, which is why it hurts less, but the source is still there and it will refill. This is the most common reason people end up in hospital with a dental infection.

Can you treat it in one visit?

The drainage, usually yes, and that is the part that stops the pain. Definitive treatment of the tooth is sometimes done the same visit and sometimes a few days later once the infection settles.
